Very important information for fans attending game at Wolves – declaration form needed

The form can be found below and this online process requires fans to confirm they have a valid NHS Covid pass or negative NHS Covid test within the previous 48 hours.

Please note, a form does NOT need to be completed ahead of Chelsea Women’s match at Kingsmeadow on Sunday.

Wolves have also confirmed they will be conducting Covid pass spot checks at designated points around the stadium ahead of kick-off, in line with the new government requirements.

Chelsea fans who attend home games will be familiar with these procedures as they have been in place at Stamford Bridge and Kingsmeadow since the start of the season.

Wolves encourage adult supporters to obtain a Covid Pass via the NHS App or NHS website so that they can provide proof of full vaccination and/or of a negative test within the 48 hours before the game, and supporters should be ready to show this when at the stadium.
